Prior to the development of prosthetic breast implants numerous materials were trialed for the purpose of augmenting the female breast. Up until the 1950's materials included autogenous fat and dermal grafts, fat injections, paraffin injections, insertion of glass balls, ivory, rubber, and terylene wool.4,5 These materials frequently led to infection, tissue necrosis and firmness of the breast. The autogenous materials were uniformily troubled by resorption. The 1950-1960's saw the use of many other products. Free injections of hydrocarbons, petroleum jelly, silicone oils, vegetable oils and bees wax were tried and led to many problems including infection, granulomas, fistulas and breast firmness.4,5 During this same time many different types of sponges were developed hoping that soft tissue ingrowth would improve the biocompatibility of these products. Polyvinyl and polyether were the most common sponge materials (figure 2) others included silastic and Teflon. Unfortunately these products were complicated by many of the same problems and were discontinued.
The development of the silicone gel prosthesis in 1962 marked a very important new era in breast surgery. Since Cronin and Gerow6 first reported its use very few other materials have been used for breast augmentation. This is partly due to the success of silicone devices and also by the subsequent FDA regulation of medical devices that was started shortly after the development of silicone gel devices.

 
 

The saline implant has an exterior sack made of solid silicone, which is filled with a sterile saline solution — like the salt-water solution that is administered for intravenous fluids. One of the advantages of this solution is its safety. Should the implant for some reason leak, the saline just gets absorbed by your body, as would the intravenous fluids used during, and after surgery. Yet another advantage of the saline implants is a very low infection rate, which is usually less than one to two percent.

 
 

Breast implants can be inserted through incisions under the breast (inframammary fold), around the areola (periareolar), or through the armpit (transaxillary). The type of implant used, the placement of incision and the size of the implant are influenced by individual physical characteristics and the patient’s preference. Silicone implants are now available! In Nov. 2006, the FDA approved the use of silicone implants for routine augmentation in women 22 years or older. Disadvantages of silicone implants are greater expense, inability to adjust size, and a longer scar to avoid damaging the implant during placement.

You still have the option of having saline implants. Should the implant rupture or leak, the saline is safely absorbed by the body and poses no health hazard. Implants may be pre-filled prior to placement, or slowly filled at the time of surgery through a self-sealing valve. Implant placement, type and size will be determined based on your breast anatomy, body type and desired increase in size, as well as your plastic surgeon's judgment. Implant manufacturers occasionally introduce new styles and types of implants; there may be additional options available to you.

Breast implants have not been shown to impair breast health. Careful review of scientific research by independent grounds such as the National Academy of Sciences Institute of Medicine (IOM) has found no proven link between breast implants and auto-immune or other systemic diseases in women. Implants can, however, create subtle or more noticeable changes in the look and feel of your breasts. Capsular contracture, a condition that causes the naturally-forming scar tissue around a breast implant to contract, occurs in a variable percentage of patients and can make the breast feel firmer than normal. While this condition can be addressed surgically, correction is not always permanent.

You should be aware that breast implants are not guaranteed to last a lifetime and future surgery may be required to replace one or both implants. Pregnancy, weight loss and menopause may influence the appearance of augmented breasts over the course of a woman's lifetime. Breast augmentation requires maintenance over time, including regular examinations for breast health and to evaluate the condition of your implants.

A mammogram may be recommended prior to your procedure to ensure breast health and serve as a baseline for future comparison. Following the procedure, mammography is technically more difficult. The Procedure
